Save Time:

Please download, fill out, and submit the following forms via email to save time and paper prior to your visit.

Policy on children:

Due to exam rooms not being a safe environment for curious children, all children 12 and under must be in a stroller or car seat to go back to the exam rooms with the patient who is being seen. Children are not allowed to wait unattended in the waiting room. Your exams require full attention by the providers and having a child in the exam room can be disruptive, unsafe and could result in a unsatisfactory examination.

We are committed to giving our patients a great experience while they are here, have the highest quality of medical care possible and to receive our full attention while under our care. If a patient arrives for his/her appointment with a child and cannot restrain them, we will have to reschedule the appointment.

Things to Remember:

Please arrive 15 minutes early for your scheduled appointment to update necessary information.

To ensure complete and thorough care, please provide us with a current list of all medications and allergies.

If you are unable to keep your appointment, please notify us as soon as possible.

We understand that your time is precious. If deliveries, emergencies, or unforeseen circumstances arise during your visit, we will inform you immediately and offer you the opportunity to see another available health care provider or reschedule your appointment.

 

Insurance & Payment

Please check with us to ensure that our health care professionals are part of your insurance provider’s network.  We accept most major insurance plans- including Medicare, Mississippi Medicaid, Blue Cross Blue Shield, Humana, United Healthcare, Aetna, Cigna, Tricare, Ambetter, Magnolia, Molina and more.

Please provide us with updated insurance information at each visit to ensure correct and timely processing. We will bill your insurance company directly for each visit.

Payment for services provided and co-pays are due at the time of the appointment.  If you have insurance questions, please call our office and speak with one of our receptionists.
